Hi all!
I have a claim in which a patient's primary insurance has dropped responsibility to patient deductible and the Medicare as secondary is denying for lack of medical necessity. I have submitted the records for proof of medical necessity and the denial has been upheld by Medicare. Would this remain, then, as patient responsibility based upon the primary insurance designation?
