Wiki Medicare Secondary Payer Form

I work in a family pratice and we were told that on every Medicare patient we have to have this filled out every time they come in. We also do labs at our office and this will create a lot of extra work and time. The form in our computer system is 5 screens. I know this isn't coding, but could someone please give me some clarification.

You should retain a copy of completed MSP questionnaires in your files or online for10 years. You may keep hard copy files, optical image, microfilm, or microfiche. If you store these files online you must keep both negative and positive responses to questions....
