Has anyone had success in filing claims over a year old with Medicare? We have situations where the patient give us the incorrect insurance info, we file the claim. The claim is paid by the carrier, only to come back a year later and recoup the money and tell us Medicare was primary. Since I wouldn't have an ICN # or denial from Medicare, would I file this as a redetermination or a clerical reopening. All responses are greatly appreciated!
