We have submitted a claim numerous times to Medicare for a Verapamil injection and it keeps coming back denied can someone please help with how this needs to be submitted to them. We sent it with the NDC#, 1 as the administered units as ML and our unit price with the J3490 procedure code. Medicare is stating we have an invalid procedure code and incorrect dosage. Please help.
