How are you billing the annual visit if they also have a sick visit and rec injections? What modifiers are used??

If you look at the ICD-10 CM codes you will see that signs and symptoms are an exclude 1 edit with the Z00.0 codes so you cannot code a sick complaint with a wellness diagnosis. This prevents you from being able to bill a sick visit with a wellness. The choice for with abnormal finding must be an abnormal finding discovered by the provider not a presenting illness.
If you have the documentation to support an abnormal finding and not a presenting complaint then the 25 modifier is what you seek.
