Would the below A/P be considered enough to count for medication management?


"A/P : Patient is a 23 yo female with hx of mood disorder , patient sustaining gains ,mood and sleep pattern stable . appetite is stable , no concerns with executive functioning . continue same meds for now .wellbutrin xl 300mg qam, vraylar 1.5mg , vyvanse 30mg .Patient did not endorse any sxs of hypomania or mania . r/b meds discussed . Pt is reporting financial hardship with monthly co payments and pymts for meds , there has not been any evidence of diversion or misuse with stimulant. pt is aware she can call next month for refill and will f/u in two months".
 
Med management has many choices...no medication, same medication, change medication, entirely new medication. If you can ascertain any of this from the note, then you've met the medication management criteria. In this case above, the provider indicates no symptoms (that might be related to medication dosage), and there were discussions with regards to risks and benefits of medication as well as the patient's concerns with cost..... all decisions for the provider as to whether to continue or change.

thank you- we have an outside auditor that didn't count this towards med management and I was confused.
