I have a few physicians who note in their E/M office visit note that "medications were reviewed with the patient as per list". In their medical decision making portion of their note, they state "continue present treatment". They keep their medication list, that is updated at every visit by the registered nurse, separate from the office notes. Is it still true that Novitas Solutions (PA MAC) requires that the physician sign and date the med list as evidence that it was reviewed that day? If so, does anyone have this information in writing? OR are the statements "medications were reviewed with the patient as per list" and "continue present treatment" evidence enough that medications were reviewed?
