Wiki Medication Usage in Pain Management Injections

If the physician performs a joint injection (CPT 20600-20610), is the medication he injects inclusive to the procedure, or would it be billed separately? Kenalog, for example.

In an office setting, the drugs are always billed separately...except for locals like Marcaine. Also don't forget the LT and RT modifiers for injections like Synvisc or Orthovisc.
