according to coding clinics when documented this way you would code it as unknown primary 199.1 and bone mets 198.5.

If that was the area being treated wouldn't mets be listed 1st?
On a side note radiology coders we only have the rad report to code from. Rarely do we have anything other than "hx of..." so we really don't know in some instances if the CA is still being treated. I know I have seen "query the provider" for additional information, again we just do not have that luxury in radiology most of the time. Especially if coders are working remotely.
