How to code these two different patients encounter Patient A-Metastatic colon carcinoma, Patient B-colon metastasis.
 
Metastatic colon cancer is something different than 'Colon metastasis', because the definition for metastasis is "The development of secondary malignant growths at a distance from a primary site of cancer."

Colon metastasis meaning secondary invasion to colon, but 'Metastatic colon cancer' is not well defined to state whether it is primary or secondary.

So I would code 153.9 for 'Metastatic colon cancer' and 197.5, 199.1 for Colon metastasis.
