Palmetto crosswalk says to replace code J9250 with J9260. The issue is CPT code J9260 is for 50mg. Essentially, we would be upcoding for a patient who receives 7.5mg of Methotrexate. How do we account for the correct dosing since we never administer 50mg of Methotrexate in Rheumatology?
