The micromanipulator CO2 laser was used to excise a small portion of the false vocal fold on the right to improve visualization. A laser incision just lateral to the true vocal fold and the micro suction was used to dissect the thyroarytenoid muscle. The laser was used to vaporize the lateral muscles, though largely replaced with fibrous tissue, reaching down to the perichondrium of the thyroid cartilage and inferiorly to the lateral cricoarytenoid muscle. The area of the entrance of the recurrent laryngeal nerve R1 branch to the thyroarytenoid muscle was lasered and cauterized using a coblator. An operating microscope was used. Is the unlisted cpt code 31599 appropriate to code for the nerve cauterization & the micromanipulator CO2 laser that was used to excise a small portion of the false vocal fold on the right? If not, what is the appropriate Cpt code? I know that I would use cpt code 31541 to capture the fibrous tissue removal.
