The NP/PA's in our hospitalist practice often visit a patient on the initial day (usually the night shift) and our admitting/supervising physician does not see them until the next day and performs the H&P.
Would the NP/PA bill a subsequent visit (99231-99233) for the initial day. Medicare does state the cpt code 99499 (other E&M code) would rarely be coded.
 
I would bill your initial visit as an initial visit 99221-99223 as the NPPs are part of the same practice as the admitting physician. If the initial services do not meet the minimum required for 99221, depending on the carrier, the advice is to bill 99499 or subsequent.

Regardless of how your initial NPP visit is billed, the visit the following day by MD/DO would be billed subsequent.
