My cardiothoracic surgeons are doing a minimally invasive Ivor Lewis esophagectomy (MIE) by laparoscopy and thoracoscopy. Also did a Botox injection lap and lap feeding jejunostomy tube.

Any suggestions on how to bill this would be appreciated.
