Wiki Minimally invasive CABG with thoracoscopy

We are having a debate with one of our surgeons. He admits that he is doing the L sided thoracoscopy as a "scout" for DaVinici; however, he feels he is not part of CABG and should be billed separately.

His op note reads "then thoracoscopy was placed into the left chest and with the thoracoscopy guidance we placed 2 xtra ports into the second and sixth intracostal space, then we placed the DaVinci instruments under vision of camera".

We disagree, we feel that this is part of the procedure and not separately billable.

His argument is that there is not a CPT code for the minimally invasive CABG yet, and if we bill it as open he is not getting the RVUs or credit that he should for the thoracoscopy.

Any assistance greatly appreciated, esp if you can provide citations!! :)
