We are a dermatology/dermatopathology practice and are CLIA certified. Our nurse manager is proposing that we start doing our own in-house testing: 36416, 80061, 81025, 84450 and 84460. Can anyone help me in where I can find good information on these tests (i.e. modifiers, diagnoses to use, etc.). Thank you.

I would start by consulting your CPT book. From my laboratory background, the tests that you listed usually do not require modifiers. They may have a Medical Necessity requirement for Medicare.
I would use 36415 for the venipuncture unless the notes specify that it was collected differently.
