I'd appreciate any direction that you could point me with this. I don't normally code surgery but was given this scenario to figure out.

This Dr. misplaced a stent and wants to know a Dx code and CPT code that could be used for removal. So, PLACED it, but actually 'misplaced' it, then had to remove it before case was done.

Looks like he used 37238 for placement - Transcatheter placement of an intravascular stent.
 
PROCEDURE:

Fistulogram right UE and SVC O Gram Balloon Angioplasty right BCV with immediate rebound. Placement of 14MM x 40MM Smart Stent in BCV with "watermelon seed phenom" requiring removal of stent with snare via right CFV and IVC-O-Gram.
