Outpatient office visit. Is is acceptable to code from a note that is missing some pages. Let's say you have pages 1,2 and 5 out of 5 page note?
I would not code it but one of the companies I work with believes it's ok to do that as long as the information on the pages that you do have supports that code.
I am trying to find any supporting documents (for whatever the answer to this question is. Which I still believe that the answer is no (you can't code off of an incomplete note)).
