A patient came in for a missionary PE and had paperwork the doctor needed to fill out. The doctor filled out the paperwork and did a full exam, ROS, and PMFSH. The doctor wants to charge a 99214, but I don't have any HPI elements, and the paperwork states that any medical conditions the patient has should be stabilized prior to attesting to the prospective missionary's medical status, so I don't see how this could be billed as a regular office visit. Should I bill 99499 and use Z02.79?

I think you meant, 99429 - Unlisted preventative service?

In our office, we get a lot of PE for summer camp, pre-employment, etc. So, we code 99429 bill w/ z02.89 dx. Check with your provider in setting the fee for the unlisted code. Hope that helps.
