I'm working a charge denial from Missouri Healthnet Medicaid. Does anyone know where on the claim this insurance carrier wants the gestational age/delivery indicator, ie 40LV on the claim?
 
This isn't Missouri Healthnet Medicaid related, but BCBS-Mississippi requires delivery indicators (SLV#1, etc.) to be entered as a line item note on the delivery CPT.
