This vaccine is now a combined vaccine for mmr and varicella. I am having a hard time on ICD-9 V code for this.. Any help would be appreciated

Two codes needed
Use V06.4 for the MMR and V05.4 for the varicella. There is a note in the V06 category stating to "use additional single vaccination code from categories V03-V05 to identify any vaccinations not included in a combnation code." Even though it is only given as one injection, both codes are needed to capture all vaccines given.
