Elderly patient seen for cognitive assessment for driver's license renewal after an MVA. MD says patient passes assessment and can continue to drive. From my research, it seems 99483 "could" be used for the MoCA assessment but the note does not support all that work described in the code, just the usual form with the clock drawing, horse/tiger/duck, physical exam, history, etc. Only bill the office visit E&M? MD is using Z02.4 as the dx but that does not seem to fit; alternative dx Z71.1?
