Oncology practice:

A pt is seen for an office visit by a provider and then has treatment therapy (chemo) by a different provider on the same day within the same practice and specialty. Would 25 modifier be added to the EM code for that day since it is two different providers?
 
IF the E/M performed is a separately identifiable service as supported by the documentation, you can use a 25 modifier. It doesn't matter that it is a different provider if they are part of the same group and billing under the same Tax ID number - the payer sees it all as the same entity.
