I am getting denied claims on preventive med with mod -25 and vaccination codes.
also I am getting denied claims with mod -25 ov's and strep test.
Can any one help me understand why all of a sudden I am getting this. I have researched and nothing is coming up.
If you have a link out there to that I can follow through I would really appreciate it.
