if a physician did a consult on the 7th and noted that likely a cath would be necessary at some point. the on the 11th he is rounding and the patient needs a cath and patient agrees can you bill the e/m and the cath for the 7th since it wasn't planned until he saw the pt that day? thanks
 
The billed date must be the date of service NOT the date it was planned or scheduled, etc. The actual date the service was performed is the date that you can bill for.
