We are billing for 99214, 99406 & 99401 in the same visit. Do I need to put a mod on each of these or just on the 99214?
 
If the E&M is separately identifiable from the preventive medicine counseling services I'd probably put modifier 25 on the E&M. Also make sure the work accounted for in the other services are not counted towards the E&M leveling.
