Wiki modifer 58

When is ok to use modifer 58? The doctor does not always list a plan of care and the patient most always returns for cysto after surgery...and it is always denied.
any imput would be appreciated

If the cysto is related to original procedure and planned, use mod 58. If unplanned use mod 78.
