What modifier would you use if billing a office visit, preventative visit and a procedure?
 
If it truly meets the billing requirements for all three it would be Modifier 25 on the Sick visit, no modifier on the preventive and mod 59 on the procedure. Of course you wouldn't bill sick and well together unless its significant and separately identifiable. you can't count work done towards the physical towards the sick visit as well. Key word significant, so if quick and easy do not report it separately. Also many procedures include a E&M component to it so the work for the sick visit may have already been included in the procedure.
