our provider is wanting to be able to upcode for difficulty with hysterectomies and the morcelletion time. i see that morcellation is included in most of the procedures. what type of documentation would be needed to justify difficulty? what is the average time for morcellation?
 
Mod 22 is very tricky. the doctor have to document all the difficulties that was encountered as well as the start and end time that it too to complete the service (eg: lysis, etc). from my experience I always have to appeal with documentation.
