Pt is in a 90 day global admitted to the hospital 99223 , also major surgery same day.Do I need to use both 24 for 90 day global and a 57 for decision for same day surgery ? UHC insurance.

Yes

Yes - if .... big, red IF ... the reason for the return to surgery is NOT related to the original surgery. If it IS related to the original surgery then the E/M will be considered global, even if it's a decision for a new surgery.
