when coding fracture care in office which modifier is applied to E&M code 25 or 57

It's best to check with your insurance carriers. Technically since the fracture care has a 90 day global you would use 57, but some insurance companies want 25 instead since you're treating the fracture in the office and the patient is not going to the OR.
