Wiki modifier 25 - explaining this right

I am a little confused on using mod 25 for , let's say 20610, and pt had an injection 5 months prior. Do I use a mod 25 again. Is there a time limit on mod 25 and injections. I hope I am explaining this right.

You would append the modifier 25 to the E&M if it was separate service on the same day, or if the service has a global of 0-10 days. Hope that answers your question.

Usually after a 5 month delay a Dr will do a separate E/M before doing the injection. But make sure the E/M can stand on it's own outside the 20610. There is some gray in this as how much of the E/M pertains to the 20610..
