Wiki modifier 25 for e/m visit with immunizations?

Hi, I am new to the coding world and am struggling with applying modifiers. I have a few providers that insist on using a modifier 25 to report a preventative visit with immunizations. I don't feel this is needed but they think I am wrong. Can anyone clarify this for me? Am I misunderstanding the definition in the book? Thanks for any help you can give me.
