Wiki modifier 25 - separate E/M was performed

I am having a tough time finding an answer to this question. I hope someone can help!! We are billing for a practice where the physical therapy was performed by a PT and a separate E/M was performed on the same date by a physician. Do we add modifier 25 since it's the same facility? Or, does modifier not apply because it's not the same physican? Any help would be greatly appreciated.

I don't think the modifier applies since the physical therapist did not do an E/M they just did the pt. The -25 only applies if a separate E/M is done by the same physician in conjunction with another service for a different problem.
 
That is how I interpreted the use of modifier 25. Thank you very much, I will see if I can convince my office of that!!
