When a provider works solely on weight management during an office visit and they spend extra time with the patient discussing preventative counseling. Would it be appropriate to use 99401-04 with modifier 25 appended to the E/M? If not please explain why you feel this way. Or can the 99401-04 code be used without the use of the modifier? Any help/guidance/resources would be most helpful!
