Wiki modifier 25 with CPT 99403

If a patient was seen for family planning visit and was given depo shot at the visit, am I able to code 99403 with a 25 modifier (for the shot)?

Hi - The rules for what's bundled and modifier use may vary based on the payer, but assuming you meet the various code requirements, you should be able to report the 99403 with 25, the injection, and the drug.
