Patient is here for sinus and congestions , shot : ceftriaxone was administered , can we add the modifier 25 to the EM code ?
 
If the documentation supports it, i.e. if it shows that there was a significant, separately identifiable evaluation and management service performed in addition to the injection. Without seeing the physician's documentation in the record, it's impossible to say if that was the case or not.
