in what circumstance would you add modifier 25 to BOTH codes?
 
none. The 25 is an E&M modifier only. It will never be use for a procedure, the provider may never bill 2 E&Ms on the same day. There is never an instance where a claim will have 2 codes both with a 25 modifier.
