At what point do we need to start using the 33 Modifier for preventative services?

Also, does this need to be appended to Medicare claims for flu shots?

The doctor is about to start doing flu shots again...last year, Medicare automatically did not assign any cost-sharing on these codes.

Am wondering if, this year, for Medicare, do we need to append the 33 Modifier?
 
I believe modifier 33 was developed for 2011, but didn't make it into the CPT book until 2012. I used it in 2011, but not all payers recognized it.

You wouldn't use this for Medicare, just private insurances.
