Wiki Modifier 33

Can anyone shed some light on Modifier 33? Do use with all insurances and does it go with all vaccines.

We have been using the 33 modifier w/commercial insurances and PT with Medicare and Medicare plans.
(We do not bill for vaccines, so I am not sure. You can always contact the specific ins. to ask their preference?)
