ED setting - Professional fee - If a reductions is performed on a shoulder by the ED provider and patient was instructed to follow up with PCP/Ortho what modifier goes on the E/M and what modifier goes on the CPT?

Depending on carrier a modifier 57 decision for surgery, or modifier 25 would go on the E/M. A modifier 54 would go on the CPT code.
