I have a general question: Many human skin substitutes such as Oasis, Dermagraft and Apligraf are utilized on a subsequent or staged basis. Is the modifier -58 appropriate to use for their applications such as CPT 15365, 15360, and such?

I know recently several MACs have released updated LCDs stating the use of -58 is inappropriate, however; several MACs still state it is appropriate with documentation.

Thoughts?
