I add facility fees and IV infusions for the ER. Should modifier 59 be appended to any IV service or only if 2 separate sites with 2 initial charges?

The need for the modifier is going to depend on what other services were performed at that encounter. You will need the modifier only if another service was billed to which an infusion is considered incidental. For example, if a surgical procedure was performed during the ER visit, then you would need a modifier 59 on any infusions that were not related to that procedure.
