Pt. goes in for a colonoscopy, scope is removed, then random bx's are taken from the ascending colon through to the rectum. A 3-mm polyp from mid-colon with hot bx is destroyed.
45384, 45380, which code should the modifier 59 be on and why?

Per NCCI edits, the 59 modifier would be applied to CPT 45380, verbage stating more extensive procedure.
