I work in Dermatology office and billing code 11601 with modifier 59 keeps getting denied when also billing Mohs code 17311-51 and repair 13132-59. This is the sequence we have.

The denial says that 59 is used incorrect with 11601 because it does not bundle... but we added modifier 59 because it doesn't bundle and the dx is different as well. Can someone help since we tried calling insurance but of course we cannot talk to coders and the representative just keep telling us they don't why
