How do you determine where to put the modifier 59 if it is not on the CMS spreadsheets?
Here is my example:
Patient had an Interlesional injection (11900) with Kenalog (J3301) for a scar. On the same visit they had a KOH done in the office (87220) for contact dermititis. Where do I put the modifier 59?
