Wiki Modifier 59 on surgery codes?

I am billing 67228 right and left at a hospital facility, do I use modifier 59 on the second line? Billing to Medicare primary.

Also, can you use 51 and 59 modifers together? I really cannot find any documentation on the Medicare website that shows this. Please help!?!
