Wiki Modifier -59 rejection by CMS

I use the -59 modifier for our chemo patient when 96416 is added at the end of the day treatment. CMS is consistently rejecting the charge because of the modifier. "Procedure code inconsistent with the modifier". Primary codes from this day are 96409, 96411, and finally the 96416. :confused:
