Pt had cath-PTA.RC and Stent.LD. then later in the day PT developed tachycardia and was brought back to the cath lab.

He did his work up and found that there was still 90% mild stenosis, distal end of the stent placed earlier in the day in the LAD.

So he did another PTA/Stent placement overlapping the previous stent in the LAD which was done earlier.

Can you code for this. Using CPT 92980.76.LD

Ok your going to think Im crazy. But listen to this. The global period for a cath is 0. meaning it spans the day of the procedure. So I have read recently that in this situation since it is still the "same day" you should add 78 modifier and in some cases 76 so it doesnt get caught up in the duplicated claim stuff!

What do you think of that?

Also dont forget you can go ahead and bill the cath placement too since it was medically necessary to take the patient back into the lab. Add 59 modifer.
