I have a laboratory that is billing 83925 with modifiers 59 and 91 instead of 83925x4. They are billing for Hydrocodone, Morphine, oxycodone and Tramedol. Would you consider this correct coding? THIS WAS NOT REPEAT TESTING. This was the initial test.
